I just ordered 500 of these order forms that I customized in about 5 minutes from the Design Aglow shop. We keep the white copy and the client receives the yellow copy with their signature approving the order, model release, and copyright legalities. All the verbiage and print resource info is included with the download. I love having simple and attractive forms!
